Military/Memorial
The following military- and memorial-themed plates are available to a select group of Tennessee residents. Unless noted otherwise, these plates cannot be personalized.
Active/Retired/Honorable Discharge
Available to: Active/Retired/Honorably discharged members and spouses of the U.S. military and military reserves in good standing. Surviving spouses also are eligible until they remarry.
Documentation Required: For active military – Current copy of Leave and Earnings Statement (LES) and stationing orders to be submitted annually upon renewal; For retired and honorable discharge military – Military ID card and one of the following in proof of status: DD Form 2A (RES), DD Form 2N (RES), DD Form 2MC (RES), DD Form 2AF (RES), DD Form 2CG (RES) or DD214.
Cost: $26.50 annual fee (initial fee is $29.80).
Air Force Cross
Available to: Recipients of the Air Force Cross.
Documentation Required: DD214 form or letter of confirmation from the Department of Veterans Services.
Cost: None for the first plate. Second plate – $26.50 annual fee (initial fee is $28.15). Subsequent plates – $26.50 annual fee (initial fee is $29.80).
Air Medal – Meritorious
Available to: Recipients of the Air Medal Meritorious.
Documentation Required: DD214 form or letter of confirmation from the Department of Veterans Services.
Cost: $26.50 annual fee (initial fee is $29.80).
Air Medal – Valor
Available to: Recipients of the Air Medal Valor.
Documentation Required: DD214 form or letter of confirmation from the Department of Veterans Services.
Cost: None for first plate. Second plate – $26.50 annual fee (initial fee is $28.15). Subsequent plates – $26.50 annual fee (initial fee is $29.80).
Blue Star Family
Available to: Person with spouse, parent, sibling or child currently serving in U.S. armed forces (i.e., Army, Navy, Air Force, Marine Corps, Coast Guard when mobilized to augment Navy, reservists and National Guard).
Documentation Required:
1. Proof of current service in the form of:
– current Leave and Earnings Statement (LES).
– member’s military ID card.
– letter from unit commander on official letterhead state family member’s current status as a member of U.S. armed forces.
2. Written statement by service member indicating the family relationship with the registrant.
Cost: $26.50 annual fee (initial fee is $29.80).
Bosnia Era Veteran
Available to: Honorably discharged veterans and civilian veterans who served in Bosnia with the U.S. armed forces, but do not qualify for the Bosnia Veteran plate.
Documentation Required: Proof of service.
Cost: $26.50 annual fee (initial fee is $29.80).
Bosnia Veteran
Available to: Honorably discharged veterans who served in Bosnia with the U.S. armed forces.
Documentation Required: Proof of service.
Cost: $26.50 annual fee (initial fee is $29.80).
Bronze Star – Meritorious
Available to: Recipients of the Bronze Star – Meritorious.
Documentation Required: DD214 or letter of confirmation from Department of Veterans Services .
Cost: $26.50 annual fee plus $29.80 initial fee (automobile); $19.50 annual fee and $22.00 initial fee (motorcycle).
Bronze Star – Valor
Available to: Recipients of the Bronze Star – Valor.
Documentation Required: DD214 or letter of confirmation from Department of Veterans Services .
Cost: None for first plate. Second plate – $26.50 annual fee (initial fee is $28.15). Subsequent plates – $26.50 annual fee (initial fee is $29.80).
Cold War Era Veteran
Congressional Medal of Honor
Available to: Recipients of the Congressional Medal of Honor.
Documentation Required: DD214 or letter of confirmation from Department of Veterans Services.
Cost: None for first plate. Second plate – $26.50 annual fee (initial fee is $28.15). Subsequent plates – $26.50 annual fee (initial fee is $29.80).
Desert Storm Era Veteran
Available to: Honorably discharged veterans and civilian veterans who served in Desert Storm with the U.S. armed forces, but do not qualify for the Desert Storm Veteran plate.
Documentation Required: Proof of service.
Cost: $26.50 annual fee (initial fee is $29.80).
Desert Storm Veteran
Available to: Honorably discharged veterans of Desert Storm.
Documentation Required: Proof of service.
Cost: $26.50 annual fee (initial fee is $29.80).
Disabled Veteran
Available to: Veterans of the United States military with a disability incurred in or aggravated by active military services of the United States.
Documentation Required: A letter from the federal Department of Veteran Affairs verifying that the veteran was rated permanent and totally disabled with an effective date.
Cost: None for first plate. Second plate – $26.50 annual fee (initial fee is $28.15). Subsequent plates – $26.50 annual fee (initial fee is $29.80).
Distinguished Flying Cross
Available to: Recipients of the Distinguished Flying Cross.
Documentation Required: DD214 or letter of confirmation from Department of Veterans Services .
Cost: None for first plate. Second plate – $26.50 annual fee (initial fee is $28.15). Subsequent plates – $26.50 annual fee (initial fee is $29.80).
Distinguished Service Cross
Available to: Recipients of the Distinguished Service Cross.
Documentation Required: DD214 or letter of confirmation from Department of Veterans Services .
Cost: None for first plate. Second plate – $26.50 annual fee (initial fee is $28.15). Subsequent plates – $26.50 annual fee (initial fee is $29.80).
Enemy Evadee
Available to: Certified enemy evadees.
Documentation Required: Discharge papers from the U.S. armed forces certified by the Department of Veterans Services. Surviving spouses can renew by providing copy of death certificate.
Cost: $26.50 annual fee (initial fee is $29.80).
5th Special Forces Group (Airborne)
Available to: Owners or lessees of passenger motor vehicles who are residents of the State of Tennessee who are active, retired, and honorably discharged members of 5th Special Forces Group (Airborne) in good standing. Spouses and children of qualifying 5th Special Forces Group members are also eligible for the plate.
Documentation Required:
– Active duty members of 5th Special Forces Group (Airborne) must present a valid military identification card or such other document as the commissioner designates as sufficient proof that the applicant is an active duty member of the military;
– Retired or honorably discharged members of 5th Special Forces Group (Airborne) must present:
(i) A certified copy of the member’s certificate of release or discharge from active duty, department of defense form 214 (DD 2114); or
(ii) A valid DD form 2 (Retired) military identification card; or
(iii) A Tennessee driver license that indicates military service
– Spouses or children of a member must be accompanied by documentation that is required from the member (as indicated above) and sufficient proof that the applicant is the spouse or child of a member, including:
(i) A valid DD form 1173 military dependent identification card; or
(ii) A marriage certificate; or
(iii) A birth certificate or certificate of adoption;
– Surviving spouses of deceased retired or honorably discharged members or of a
member who died while on active duty shall be issued a license plate pursuant to this
section, until the surviving spouse remarries, upon providing documentation required from the member (as indicated above) and providing a copy of the death certificate of the deceased.

Gold Star Family
Available to: Person with spouse, parent, sibling or child who served and died in the line of duty in the U.S. armed forces or its allies, or died as a result of injuries sustained in such service.
Documentation Required: DD Form 1300 Report of Casualty.
Cost: None for first plate. Additional plates – $26.50 annual fee (initial fee is $29.80).
Honorably Discharged
Available to: Honorably discharged veterans of the U.S. armed forces.
Documentation Required: Proof of service.
Cost: $26.50 annual fee (initial fee is $29.80).
Korean War Era Veteran
Available to: Honorably discharged veterans and civilian veterans who served in the Korean War with the U.S. armed forces, but do not qualify for the Korean War Veteran plate.
Documentation Required: Proof of service.
Cost: $26.50 annual fee (initial fee is $29.80).
Korean War Veteran
Available to: Honorably discharged veterans of the Korean War.
Documentation Required: Proof of service.
Cost: $26.50 annual fee (initial fee is $29.80).
National Guard
Available to: Officers, enlisted members and retired/honorably discharged members of the Tennessee National Guard, along with their spouses.
Documentation Required: County clerks have eligibility lists. Guard members not listed must provide documentation with name, social security number and proof of good standing to qualify for plate. Retired and honorably discharged applicants must provide current and valid armed forces of Tennessee card stating that status.
Cost:
– Active officers/spouses* – $26.50 annual fee.
– Enlisted members and spouses* – None for first plate; Annual fee of $26.50.
– Retired and honorably discharged members/spouses – $26.50 annual fee (initial fee is $29.80).
*NOTE: Officer plates issued in September only; expire Sept. 30.
Navy Cross
Available to: Recipients of the Navy Cross
Documentation Required: DD214 or letter of confirmation from the Department of Veteran Services.
Cost: None for the first plate. Second plate – $26.50 annual fee (initial fee is $28.15). Subsequent plates – $26.50 annual fee (initial fee is $29.80)
Operation Enduring Freedom Era
Available to: Honorably discharged veterans and civilian veterans who served during Operation Enduring Freedom with the U.S. armed forces, but do not qualify for the Operation Enduring Freedom Veteran plate.
Documentation Required: Proof of service.
Cost: $26.50 annual fee (initial fee is $29.80).
Operation Enduring Freedom
Available to: Honorably discharged veterans of Operation Enduring Freedom or active members currently serving in Operation Enduring Freedom.
Documentation Required: Active members – Award certificate indicating receipt of campaign ribbon (Afghanistan); Discharged members – DD214 form indicating receipt of campaign ribbon (Afghanistan).
Cost: $26.50 annual fee (initial fee is $29.80).
Operation Iraqi Freedom Era
Available to: Honorably discharged veterans and civilian veterans who served during Operation Iraqi Freedom with the U.S. armed forces, but do not qualify for the Operation Iraqi Freedom Veteran plate.
Documentation Required: Proof of service.
Cost: $26.50 annual fee (initial fee is $29.80).
Operation Iraqi Freedom
Available to: Honorably discharged veterans of Operation Iraqi Freedom or active members currently serving in Operation Iraqi Freedom.
Documentation Required: Active members – Award certificate indicating receipt of campaign ribbon (Iraq); Discharged members – DD214 form indicating receipt of campaign ribbon (Iraq).
Cost: $26.50 annual fee (initial fee is $29.80).
Operation New Dawn Era
Available to: Honorably discharged veterans and civilian veterans who served during Operation New Dawn with the U.S. armed forces, but do no qualify for the Operation New Dawn Veteran plate.
Documentation Required: Proof of service.
Cost: $26.50 annual fee (initial fee is $29.80).
Operation New Dawn
Available to: Honorably discharged veterans of Operation New Dawn, active members currently serving in Operation New Dawn and civilian veterans of the U.S. Army Corps of Engineers.
Documentation Required: Orders or DD214 form confirming honorable discharge, or orders or official documentation from the Army Corps of Engineers classifying the applicant as a civilian veteran.
Cost: $26.50 annual fee (initial fee is $29.80).
Paratrooper
Available to:
Active/Retired/Honorably discharged members of the United States armed forces, and who, as part of the member or veteran’s military duty, has been trained to jump from flying aircraft.
Documentation Required:
– (1)(A) – A valid military identification card or such other document as the commissioner designates as sufficient proof that the applicant is an active member of the United States armed forces; or
– (1)(B) – A certified copy of the veteran’s certificate of release or discharge from active duty, department of defense form 214 (DD 214), honorable discharge certificate, department of defense form 256 (DD 256), or report of separation and record of service, NGB form 22, that indicates an honorable discharge characterization;
– A valid DD form 2 (Retired) military identification card; or
– A Tennessee driver license that indicates military service in accordance with § 55-50-354; and
– (2) Documentation of satisfactory completion of the prescribed proficiency tests while assigned or attached to a military airborne unit or the airborne department of the United States army infantry school, to include:
(A) – Military orders awarding the parachutist badge authorized by the United States army to the member or veteran;
(B) – The veteran’s certificate of release or discharge from active duty, department of defense form 214 (DD 214), or report of separation and record of service, NGB form 22, indicating the awarding of the parachutist badge; or
(C) – The airborne course diploma issued to the member or veteran by the United States army infantry school.

Pearl Harbor Survivor
Available to: Tennessee residents who were members of the armed forces and present at Pearl Harbor on December 7, 1941.
Documentation Required: Copy of membership card from the Pearl Harbor Survivor’s Association, Inc.
Cost: $26.50 annual fee (initial fee is $29.80).
POW – Former Prisoner of War
Available to: Former World War I, World War II, Korean War and Vietnam War POWs, along with surviving spouses until remarriage.
Documentation Required: Certificate from U.S. Department of Defense stating the period of time the applicant was a POW. Death certificates required for spouses of deceased POWs.
Cost: None for first plate. Second plate – $26.50 annual fee (initial fee is $28.15). Subsequent plates – $26.50 annual fee (initial fee is $29.80).
Purple Heart
Available to: Holders of The Purple Heart Medal.
Documentation Required: DD214 form or its equivalent prior to 1951 (WWK 53-55) indicating award of Purple Heart, or other documentation indicating the award (to be submitted for approval by Veterans Affairs prior is issuance).
Cost: None for first plate. Second plate – $26.50 annual fee (initial fee is $28.15). Subsequent plates are $26.50 annual fee (initial fee is $29.80).
* Disabled veteran recipients of Purple Heart may obtain two plates. Cost: None for first plate if chosen as an alternative to a Disabled Veteran plate. Additional plate: $26.50 annual fee (initial fee is $28.15).
Rakkasans
Available to: Active, retired, and honorably discharged member of the Rakkasans, which is the moniker for the 187thInfantry regiment of the 101st Airborne Division (Air Assault) of the United States Army, in good standing.
Documentation Required:
Military orders or other official documentation detailing an assignment to the 187th Infantry Regiment of the 101st Airborne Division (Air Assault) of the United States Army and
The following documentation must be provided:
Active duty members must present:
A valid military identification card or such other document as the commissioner designates as sufficient proof that the applicant is an active duty member of the military
Retired or honorably discharged members must present:
Certified copy of the member’s certificate of release or discharge from active duty, department of defense form 214 (DD 214)
Valid DD form 2 (Retired) military identification card; or
Tennessee driver license that indicates military service
Spouses or children of a member must submit the following:
Documentation that is required from the member (as indicated above) and
Sufficient proof that the applicant is the spouse or child of the member, including:
i.) Valid DD form 1173 military dependent identification card; or
ii.) Marriage certificate; or
iii.) Birth certificate or certificate of adoption

Silver Star Valor Redesign
Available to: Silver Star recipients.
Documentation Required: DD214 form or letter of confirmation from the Department of Veterans Services.
Cost: None for first plate. Second plate – $26.50 annual fee (initial fee is $28.15). Subsequent plates – $26.50 annual fee (initial fee is $29.80).
Submarine Veteran
Available to: Honorably discharged veterans who served on a Department of Veterans Services-certified submarine in the line of military duty.
Documentation Required: DD214 form indicating “SS” (Submarine Service) after the ranking.
Cost: $26.50 annual fee (initial fee is $29.80).
Support Our Troops
Available to: Tennessee residents.
Proceeds Benefit: Support Our Troops Inc.’s programs and activities in Tennessee.
Cost: $61.50 annual fee, $35 of that fee is allocated in part to the plate’s beneficiary.
Option to Personalize: Yes, between three and five characters.
Vietnam Era Veteran
Available to: Honorably discharged and civilian veterans who served in the Vietnam War with the U.S. armed forces, but do not qualify for the Vietnam War Veteran plate.
Documentation Required: Proof of service.
Cost: $26.50 annual fee (initial fee is $29.80).
Vietnam Veteran
Available to: Honorably discharged Vietnam War veterans.
Documentation Required: Proof of service.

Tennessee Woman Veteran
Available to: Females honorably discharged from the armed services.
Documentation Required: Copy of DD214 form.

World War II Era Veteran
Available to: Honorably discharged veterans and civilian veterans who served in World War II with the U.S. armed forces, but do not qualify for the World War II Veteran plate.
Documentation required: Proof of service.

World War II Veteran
Available to: Honorably discharged World War II veterans.
Documentation required: Proof of service.
