Commonly Asked Items for FY2019 Adaptive Sports Grant Application
The following items are commonly asked about for the FY2019 Adaptive Sports Grant (ASG) application.
The ASG does not use additional Applicant Identifier, Federal Entity Identifier, and Federal Award Identifier designations, therefore Blocks 4, 5a and 5b in the SF-424 can be left blank.
Executive Order 12372 does not apply to the ASG program, therefore Blocks 6 & 7 in the SF-424 can be left blank and Block 19 is marked c. Program is not covered by E.O. 12372.
The VA Administrative and Financial Review Questionnaire for Grantees is not required for the FY19 ASG application as it has been discontinued by VA pending administrative review actions.
The separate Abstract cited in the SF-424 is not required because the “Narrative” document cited in Section IV.B. of the FY19 ASG Notice of Funding Availability (NOFA) includes that data.
The separate Program Past Performance and Program Specifics documents cited in the VA Form 10096 are not required as separate documents as that data can be included in the “Narrative” document cited in Section IV.B. of the FY19 ASG NOFA.
All applicants are required to include the VA Form 10091 in the FY19 ASG application..
Titles for attachments do not need to be longer than those provided in Section IV.B. of the FY19 ASG NOFA, because all documents are accessed through a compressed file in Grants.gov and long titles actually cause problems in data management.
All FY19 ASG applications are “New” applications as this is a one-year grant. Therefore, Block 2 in the SF-424 is always “New”.
As explained in Section IV.B. of the FY19 ASG NOFA, the letter of support should be submitted as either Letters of Outreach Support or Letters of Partnership. Divide letters of support into the most appropriate category and combine these into one attachment for each category, either through scanning together or some other means that works.
